NOHN Receives Grant Funding for Care Coordinators

We’re pleased to be a recipient of grant funding from PacificSource Health Plans as part of its annual Community Health Excellence (CHE) grants program for the 2023-2024 cycle. Now in its 14th year, the CHE program has awarded more than $11 million in community grant awards to providers advancing healthcare delivery innovations in Washington, Idaho, Montana, and Oregon.

NOHN received $78,000 to expand its team of full-time care coordinators from two to four. Care coordinators play a vital role on a patient’s clinical care team by managing transitions of care, addressing care gaps, identifying social drivers of health, and integrating additional services such as patient navigation, behavioral health, oral health, pharmacy, and optometry. In addition to increasing the team’s overall capacity to proactively identify and assist high-risk patients, the new hires will focus on transitional care management for patients recently discharged from the ER.

“Care coordination is key to improving health outcomes for our underserved and elderly patients, particularly those with chronic conditions,” said NOHN CEO Michael Maxwell, MD. “We’re grateful for the funding we’ve received from PacificSource to better serve these patients and pleased to welcome Tiana Taylor, MA, to the NOHN family as our first PacificSource grant-supported hire.”

Applications for CHE program funding are independently evaluated, and awards go to organizations that demonstrate significant positive impact for their patients while prioritizing the Quadruple Aim of enhancing the patient experience, improving population health, reducing costs, and improving the wellbeing of the care team.
