NOHN at AHA Hypertension Summit

NOHN was among a select group of Washington State healthcare organizations presenting at the American Heart Association’s Puget Sound Hypertension Summit on Wednesday, October 30 at the Hilton in Bellevue.

Kate Weller, MD, Chief Medical Officer, Terri Hauff, LPN, Senior Clinical Risk Manager, and Carlos Osorio, Outreach and Navigation Manager, presented “Utilizing Community Health Workers in a Self-Measured Blood Pressure Program at an FQHC”. Dr. Weller was also part of a moderated panel for the summit’s closing discussion.

In addition, the team received the AHA’s 2024 Target:BP Gold Award on behalf of NOHN. The award is given to practices that have helped control blood pressure in at least 70% of adult hypertensive patients. Congratulations to our clinical and quality teams, and everyone who contributed to this accomplishment!
